3 Month Contract With A Local Authority


Job Purpose:

To provide high-quality, person-centred social work to adults with care and support needs within the community. As part of the Adult Neighbourhood Team, the Social Worker will be responsible for undertaking strengths-based assessments, support planning, safeguarding, and multidisciplinary work to promote independence, wellbeing, and safety in line with the Care Act 2014.



Key Responsibilities:

  • Conduct strengths-based Care Act assessments, risk assessments, and outcome-focused support plans for adults with physical, mental health, or learning disabilities.

  • Carry out reviews of existing care packages to ensure they remain appropriate, cost-effective, and aligned with the individual’s current needs.

  • Respond to and manage safeguarding concerns in accordance with Section 42 of the Care Act and the council’s safeguarding policies.

  • Work collaboratively with health partners, housing, voluntary organisations, and internal services to deliver holistic support.

  • Promote early intervention and preventative approaches, making use of community resources and reablement services.

  • Maintain accurate and timely case records using the local authority’s case management system.

  • Participate in team meetings, supervision, and case discussions, contributing to the ongoing development of good practice.

  • Ensure legal and policy compliance across all areas of work, including Mental Capacity Act, Human Rights Act, and Data Protection.


Requirements:

  • Qualified Social Worker with SWE registration.

  • Experience in adult social care, particularly in community-based or multidisciplinary settings such as an Adult Neighbourhood Team.

  • Strong assessment, communication, and report-writing skills.

  • Ability to manage a diverse caseload and prioritise competing demands effectively.

  • Enhanced DBS (preferably on the update service).

  • Experience using electronic case management systems (e.g., Liquidlogic, Mosaic, CareFirst – depending on the authority).
