2 Month Contract With A Local Authority
Job Purpose
To work within day care services, supporting adults with learning disabilities, physical disabilities, and complex health needs. This role is based across various community settings and involves implementing meaningful, person-centred activities to enhance individual independence, life skills, and social interaction. The role includes both physical and emotional support, tailored to each individual’s care and development needs.
Key Responsibilities
Provide personal care, support with hydration and nutrition, and assistance with medication in line with individual care plans.
Always adhere to adult safeguarding procedures to ensure the safety and well-being of those receiving support.
Develop and deliver individualised and group activities using a person-centred approach.
Maintain and prepare appropriate resources for activities, ensuring they are inclusive and meaningful.
Observe, monitor, and promote the health and well-being of individuals both in day centres and while out in the community.
Highlight any changes in health or behaviour to the relevant team member to ensure care plans are updated accordingly.
Contribute to care and support plans by providing feedback and observations.
Accurately complete written records, logs, and reports using both paper-based and electronic systems.
Follow financial procedures when making purchases on behalf of the service.
Promote independence in managing money, ensuring receipts are obtained and transactions are logged appropriately.
Comply with all policies regarding the recording, sharing, and secure storage of sensitive and personal information.
Maintain professional, respectful relationships with service users, their families, carers, colleagues, volunteers, and external stakeholders.
Attend meetings such as supervisions, team meetings, reviews, and multidisciplinary planning sessions.
Requirements
Knowledge of person-centred care and support approaches for adults with disabilities.
Experience providing care or support in any social care context (paid, voluntary, or personal).
Understanding of health and safety responsibilities in a care environment.
Ability to communicate clearly and professionally with service users, families, and team members, both verbally and in writing.
Level 2 qualification in Health and Social Care (or willingness to work towards it).
Experience supporting adults with behaviours of concern.
Experience supporting individuals with complex health needs.
Understanding of best practices in community inclusion and activity planning.
