1 month contract with a local authority (TBC)

Job Purpose
  • The reviewing officer's role in adult social care hospital teams is vital for ensuring timely and suitable transitions for individuals from hospital to their home or other care environments. This requires a thorough understanding of the individual's needs, their existing support networks, and accessible community resources.

Key Duties/Accountabilities
  • Assessment and Reassessment: Performing comprehensive evaluations of individuals' social care needs upon admission to the hospital and at key points during their stay. This includes assessing their physical health, mental health, cognitive abilities, daily living skills, and social circumstances.
  • Care Planning and Coordination: Creating personalised care plans in partnership with the individual, their families, hospital staff (doctors, nurses, therapists), and other relevant professionals. This involves identifying suitable care options, such as home care, residential care, or rehabilitation services.
  • Discharge Planning: Leading or actively participating in discharge planning meetings to ensure safe and timely discharges. This includes organising necessary equipment, adaptations, and ongoing care services to support the individual's transition.
  • Safeguarding: Identifying and responding to safeguarding concerns to protect the individual's safety and well-being. This may involve liaising with adult safeguarding teams and other statutory bodies.
Essential Experience Required
  • Strong communication and interpersonal skills: To effectively engage with individuals, families, and a diverse range of context professionals.
  • Excellent assessment and analytical skills: Able to accurately identify needs and develop suitable care plans.
Essential Qualification Required
  • Education qualification in the related field is essential.
