Job description
1 Year contract with a local authority
Summary
- The Leaving Care Service is dedicated to providing statutory services to care leavers aged 18 and above. Personal Advisors play a crucial role in extending these services to care leavers until they reach the age of 25. The service is managed by Consultant Social Workers or Practice Development Managers, who ensure that the Local Authority maintains contact with care leavers, facilitates access to suitable accommodation, and provides opportunities for education, training, and employment.
Responsibilities
- Establish rapport and build respectful relationships with care leavers, their families, and support networks.
- Undertake Review Pathway Plans and coordinate support interventions, ensuring care leavers' views and wishes are accurately recorded.
- Engage and communicate effectively with other agencies, incorporating their professional views into care plans.
- Work collaboratively and maintain regular liaison in the best interests of care leavers.
- Monitor and assess risk in care leavers' accommodation or other settings, implementing agreed support plans.
- Offer practical help to enable care leavers to become self-sufficient and support them in developing their roles as young parents or carers, where relevant.
- Work independently, making decisions about necessary tasks and taking responsibility for their impact on care leavers.
Requirements
- Relevant and substantial experience in direct work with children, young people, and families.
- NVQ Level 3 or equivalent child and family training.
- A satisfactory Disclosure and Barring Service (DBS) check at enhanced level is required.
