3 months contract with a Local Authority
Job Summary:
• The Outreach Team Leader will be responsible for delivering high-quality, person-centred services that promote wellbeing, independence, and healthy lifestyles for vulnerable adults.
• The role involves supervising and supporting staff, ensuring compliance with CQC fundamental standards, and providing effective leadership to achieve positive outcomes for service users.
• The post holder will deputise for the manager as required and contribute to the smooth and efficient running of the service in line with policies, procedures, and legislation.

Key Duties/Accountabilities (Sample):
• Lead, supervise and support outreach staff to deliver safe, person-centred support to vulnerable adults.
• Oversee care planning, assessments, and risk management, ensuring services are outcome-focused.
• Promote wellbeing, independence, inclusion, and choice for service users through innovative and flexible approaches.
• Undertake staff supervision, performance monitoring and rota management within budget.
• Ensure compliance with CQC standards, safeguarding procedures, and council policies.
• Develop and maintain effective partnerships with carers, families, professionals, and external agencies.
• Contribute to continuous improvement through staff development, training, and service innovation.
• Maintain accurate records and reports using electronic systems, ensuring data protection and confidentiality.
• Participate in the on-call rota as required and deputise for the manager when necessary.

Skills/Experience:
• Strong knowledge of adult social care legislation and safeguarding principles.
• Proven experience supporting vulnerable adults, including those with complex or challenging needs.
• Ability to supervise, motivate and develop staff within a performance management framework.
• Experience in developing and reviewing personalised care plans and risk assessments.
• Excellent organisational and time management skills, with the ability to prioritise workloads effectively.
• Strong IT and record-keeping skills, with experience using care management systems.
• Confident communicator, able to build effective relationships with internal and external partners.
• Flexible approach with the ability to participate in out-of-hours/on-call duties.

Additional Information:
• The closing date: 20/10/2025.
• Vocational qualification equivalent to NVQ Level 4 or above (or equivalent experience).
• Full-time temporary position (35 hours per week).
