Job description

3 months contract with local authority

We are seeking a dedicated individual to join our Islington Reablement Service team, providing intensive, short-term support to enable individuals to regain or maximize their independence. As an Enabler, you will work with adults with a range of needs, including learning disabilities, physical and sensory needs, mental health, and older people, across various community and hospital settings.

Key Responsibilities:
  • Delivering personalized care and support in accordance with individuals' Reablement plans
  • Working closely with health and social care professionals to ensure effective care planning and delivery
  • Demonstrating best practice in strengths-based and person-centered approaches
  • Collaborating with residents, carers, and community-based organizations to promote independence
  • Adhering to the Council's policies and procedures, including safeguarding and the Care Act 2014
  • Providing support with medication, nutritional needs, and specific dietary requirements
  • Maintaining accurate records and documentation in compliance with organizational standards
  • Ensuring infection control and legal, regulatory, and policy compliance
  • Participating in multi-disciplinary meetings and contributing to service outcomes and personal development


Requirements


  • Willingness and ability to travel across the team's geographical area
  • Flexibility to work over a 7-day working week, including morning, evening, and overnight shifts
