3-Month Contract with a Local Authority

Summary
  • This role provides statutory support and guidance to care-experienced young people aged 16–21 (or up to 25 where applicable), ensuring they receive the necessary advice, resources, and skills for a successful transition to independent living. As the key professional and case manager for young people leaving care, this position is crucial in promoting their well-being and independence.
Responsibilities
  • Undertake the statutory role of Personal Adviser for care-experienced young people aged 16–21 (or up to 25).
  • Act as the key professional and case manager for Former Relevant young people and, where required, for Relevant or Qualifying young people.
  • Collaborate with young people and their support networks to develop independent living skills and ensure a smooth transition from care to independence.
  • Provide practical and emotional support, advice, and guidance to young people preparing for leaving care and living independently.
  • Assist young people in accessing education, training, employment, housing, and health services.
  • Maintain accurate records and ensure compliance with statutory requirements and local policies.
  • Promote safeguarding and well-being in all interactions.
Essential Experience Required
  • Previous experience working with young people in a support or advisory capacity.
  • Experience in multi-agency working and case management.
  • Familiarity with statutory responsibilities for care leavers is advantageous.
Essential Qualifications Required
  • Good standard of education (minimum Level 3 qualification or equivalent).
  • Strong interpersonal and communication skills.
  • Ability to build positive relationships with young people and professionals.
  • Knowledge of care leaver legislation and guidance.
  • Ability to work independently and manage a caseload effectively.
  • Full UK driving license, access to own vehicle, and business insurance.
Additional Information
  • Working hours: 37 hours per week
  • Location: Darby House, Lawn Central, Telford, TF3 4JA & Leavers Lounge, Meeting Point House, Southwater, Telford, TF3 4HA
  • Hybrid working arrangement with office presence approximately twice a week.
  • Travel across Telford and surrounding areas required.
  • Enhanced DBS required
