We are seeking a dedicated and compassionate Family First Case Manager to join our team in the Social/NGO/Health & Care industry. The ideal candidate will be responsible for providing comprehensive case management services to families in need, ensuring that they have access to the resources and support necessary to thrive. The Family First Case Manager will work closely with individuals and families to assess their needs, develop personalized care plans, and connect them with community resources and services. This role requires a strong commitment to advocating for the well-being of families and the ability to collaborate effectively with other professionals in the field.

Responsibilities

  • Conduct comprehensive assessments of families to identify their needs and challenges.
  • Develop and implement individualised care plans to address the specific needs of each family.
  • Provide ongoing support and guidance to families, connecting them with relevant resources and services.
  • Advocate for families and ensure that they have access to the necessary support systems.
  • Collaborate with other professionals, including social workers, healthcare providers, and community organizations, to coordinate services for families.
  • Maintain accurate and up-to-date case records and documentation
  • Monitor and evaluate the progress of families and adjust care plans as needed.
  • Participate in team meetings and case conferences to discuss complex cases and share best practices

Requirements

Requirements:
  • Bachelor's degree in social work, psychology, or a related field
  • Proven experience in case management, preferably in a social/NGO/healthcare setting
  • Strong understanding of community resources and support services
  • Excellent communication and interpersonal skills
  • Ability to work effectively with individuals and families from diverse backgrounds
  • Compassionate and empathetic approach to supporting families in need
  • Strong organizational and time management skills
  • Ability to collaborate with a multidisciplinary team
  • Knowledge of relevant laws and regulations related to family services and support
