We are seeking a dedicated individual to join our project team, the Progression and Next Steps Team, within the Learning Disability Service. As a Learning Disability Social Worker, you will play a crucial role in completing reassessments of individuals in residential care settings, with the aim of transitioning them into less restrictive care environments such as supported living or shared lives. Additionally, you will be responsible for upskilling individuals within care settings to reduce reliance on commissioned care. The successful candidate will need to demonstrate expertise in strength-based assessments, evidence-based support, and negotiating costs. This role also requires legal literacy to undertake Mental Capacity Act (MCA), Best Interests (BI), and Court of Protection (CoP) work. The candidate will be expected to achieve savings targets attached to the project.
