We are seeking a Mental Health Social Worker to fulfill responsibilities including completing Care Act Assessments, Reviews, Safe guarding, and care coordination under the CPA Framework. The successful candidate will manage a caseload of 25 clients and be an integral part of a Duty Rota, which may involve screening, triaging, and handling new referrals. This role will also involve active participation in an integrated Mental Health and Wellness Team.
