3 months contract with local authority

Responsibilities

• Conduct outreach throughout the Walsall borough to locate rough sleepers and follow up on referrals.
  
• Perform holistic risk and needs assessments, developing support and risk management plans.
  
• Design and implement life and social skills programs for service users in both individual and group settings.
  
• Encourage service users to engage in learning skills and work opportunities.
  
• Support service users in developing positive family and support networks.
  
• Assist service users in finding settled accommodation and provide ongoing support.
  
• Be available for regular support sessions and respond to crises.
  
• Safeguard the welfare of service users.
  
• Promote and facilitate service user participation in their support plans and service management.
  
• Facilitate access to and uptake of support services for service users.
  
• Ensure benefit claims are made and maintained to maximize service users' income.
  
• Organize and accompany service users to appointments as needed.
  
• Maintain accurate records, including support plans and outcomes.
  
• Be flexible and available out of hours, including weekends and bank holidays, on a rota basis to provide information, advice, and guidance to colleagues.
  
• Assist in the induction of new staff and supervise and support volunteers, trainees, and students as required.
  

• Commitment to supporting vulnerable service users.
  
• Valid driving license and access to a vehicle for work use.
  
• Experience in supporting individuals with complex needs.
  
• Ability to conduct risk and needs assessments.
  
• Strong organizational and communication skills.
  
• Flexibility to work out of hours, including weekends and bank holidays.
