3-Month Contract with a Local Authority
Summary
- This role involves delivering high-quality, person-centred social work interventions within a dynamic, multi-disciplinary team. The primary objective is to support individuals in crisis, aiming to prevent hospital admissions and promote community-based care.
Responsibilities
- Conduct strengths-based assessments (SBA) and co-create care plans with individuals using a person-centred approach.
- Address urgent social care referrals, safeguarding concerns, and crisis situations.
- Collaborate with a multi-professional team, including nurses, occupational therapists, physiotherapists, GPs, and support staff.
- Provide timely social care packages and contribute to medical and nursing interventions.
- Participate in the Virtual Ward and other referral pathways (e.g., 111, GPs, D2A, LAS).
- Complete Continuing Health Care (CHC) assessments for individuals without allocated social care staff.
- Work one late shift per week (12:30 PM – 8:30 PM) and one weekend in four (with Thursday and Friday off that week).
- Maintain accurate records and contribute to case management and supervision.
- Engage in ongoing professional development and training provided by the team.
Essential Experience Required
- Proven experience as a qualified Social Worker in a health or social care setting.
- Strong understanding of crisis intervention and safeguarding procedures.
- Experience in multi-disciplinary team working.
- Desirable: Experience in completing CHC assessments (training will be provided if needed).
- Ability to work flexibly, including evenings and weekends.
Essential Qualification Required
- Degree in Social Work or equivalent.
- Registration with Social Work England.
- Enhanced DBS clearance.
- Evidence of continued professional development.
Additional Information
- Working hours: 35 hours per week.
- The role requires an Enhanced DBS.
