Job description
3 months contract with local authority
The role involves providing personalized care and support to individuals based on their tailored support or Reablement plans. This includes performing personal care tasks, domestic duties, or exercise programs designed by therapists to help individuals achieve their rehabilitation or recovery goals at home. The focus is on enabling and promoting independence. The Enabler will collaborate closely with health and social care professionals involved in the care planning and delivery of Reablement services, reporting directly to the Reablement Senior Enabler/Coordinator.
Responsibilities:
- Deliver personal care and support in line with personalized Reablement plans.
- Perform personal care tasks, domestic duties, or exercise programs as needed.
- Support individuals in achieving their rehabilitation or recovery goals at home.
- Promote independence and enablement through care and support.
- Collaborate with health and social care professionals involved in care planning.
- Report to the Reablement Senior Enabler/Coordinator.
